STARS Center Plan Template
It is recommended that centers use this template to begin to implement STARS. Formally-designated STARS centers will be required to complete this template.
Volunteer Application
A potential volunteer should complete this form in order to be considered for a mentoring and/or tutoring partnership with a Job Corps student. The form should be submitted to the STARS Lead.
Tutor Interview Template
Centers should use this form to interview a potential tutor.
Mentor Interview Template
Centers should use this form to interview a potential mentor.
Sample Orientation Letter
This letter advises a volunteer of his/her acceptance as a STARS volunteer at the designated Job Corps center. It informs the volunteer of a scheduled center orientation date.
Speaker Sign-up Form
This form should be completed by individuals interested in becoming a local speaker at a Job Corps center. It should be reviewed by the STARS Lead to determine the next step in the speaker interview process.
Speaker Interview Template
This template should be used by centers to interview potential speakers.
Speaker Event Day Information Sheet
This Information Sheet provides a speaker with all the information he/she will need in advance of the speaking engagement, such as date, location, number of students attending, etc.
STARS Volunteer Reference Sheet
This sheet should be used by the STARS Lead when verifying and interviewing a potential volunteer's personal and professional references.
STARS Tutoring Partnership Agreement
This Partnership Agreement should be completed by the tutor and the student to establish their tutoring relationship.
STARS Mentoring Partnership Agreement
This Partnership Agreement should be completed by the mentor and the student to establish their mentoring relationship.
STARS Volunteer Confidentiality Agreement
This agreement is signed by a volunteer to promise to hold in confidence all information regarding youth involved in STARS.
STARS One-Pager
This document provides a one-page overview of STARS.
STARS Key Messages
This document provides key information regarding STARS and its goals and purpose and the requirements for speakers, mentors and tutors.
STARS PSAs
Centers are encouraged to contact local radio stations to inquire about opportunities to recruit STARS volunteers. This document provides sample Public Service Announcements.
STARS Press Release
The attached file contains a sample Press Release regarding STARS. It may be modified to adapt to each center.

This PowerPoint provides an overview of STARS. Centers may wish to use this with center staff as well as in the community to recruit volunteers.
STARS Recruitment Flier
Centers may customize this Recruitment Flier to best suit their needs. Fliers may be posted on bulletin boards in various locations near the centers as a recruiting tool. Centers may also wish to distribute fliers at networking functions, community meetings or other information sessions.
